About the Book

Der Wilms-Tumor ist der häufigste Nierentumor im Kindesalter und besonders im Kleinkindesalter. Die ersten Berichte über diese maligne Nierengeschwulst bei Kindern sind über 150 Jahre alt. Es sind zu nennen Rance (1814), Gairdner (1828), Eberth (1875), der einen umfassenden Bericht gab, Cohnheim (1875). 1877 führte Jessop die erste Tumorexstirpation bei einem 2jährigen Mädchen durch; die 2. Operation wurde im Jahre 1888 von Israel vorgenommen. Birch-Hirschfeld und Wilms haben 1898 und 1899 die als klassisch bekannten Arbeiten veröffentlicht. Seitdem spricht man zumindest im deutschsprachigen Raum vom Wilms-Tumor. Aus der Tatsache, daß es in der Literatur mindestens 48, nach Sigel sogar 53 ver- schiedene Namen gibt, läßt sich schließen, daß die morphologische und histologische Struktur dieses Tumors sehr vielgestaltig sind. Es liegt auch die Folgerung nahe, daß die Ontogenese aus eben diesen Gründen nicht eindeutig geklärt ist. Einen Überblick darüber bringt Sigel im Lehrbuch der Kinderurologie. Man kann sagen, daß der Wilms-Tumor zu den embryonalen Geschwülsten gehört, die als ein Teil der Organanlage maligne ent- arten. Dabei ist der Zeitpunkt der Entstehung nicht genau definiert. Der Wilms-Tumor dürfte aber parallel der Nachnierenentwicklung in der Embryonalzeit entstehen. Dafür spricht, daß in einer Reihe von Beobachtungen der Tumor bereits in der Neugeborenen- Periode gefunden wird. Als Ursache der Tumorentstehung werden vorwiegend exogene Noxen diskutiert, wie ionisierende Strahlen und Chemikalien, die transplazentär über- treten oder nach der Geburt verabreicht werden. 2. Disposition, Häufigkeit, Seitenverteilung, Alters- und Geschlechtsverteilung, familiäres Vorkommen Eine konstitutionelle Disposition wird vereinzelt angenommen.
