Avant de l'écrire, les frères Goncourt ont passé plusieurs jours dans les salles d'hôpitaux.Les scènes du roman retracent la vie à l'hôpital, les conversations des malades, les visites du médecin en chef.ExtraitLa salle est haute et vaste. Elle est longue, et se prolonge dans une ombre où elle s'enfonce sans finir.Il fait nuit. Deux poêles jettent par leur porte ouverte une lueurrouge. De distance en distance, des veilleuses, dont la petite flamme décroît à l'oeil, laissent tomber une traînée de feu sur le carreau luisant. Sous leurs lueurs douteuses et vacillantes, les rideauxblanchissent confusément à droite et à gauche contre les murs, des lits s'éclairent vaguement, des files de lits apparaissent à demi que la nuit laisse deviner. A un bout de la salle, dans les profondeurs noires, quelque chose semble pâlir, qui a l'apparence d'une vierge de plâtre.L'air est tiède, d'une tiédeur moite. Il est chargé d'une odeur fade, d'un goût écoeurant de cérat échauffé et de graine de lin bouillie. Tout se tait. Rien ne bruit, rien ne remue. La nuit dort, le silence plane. A peine si, de loin en loin, il sort de l'ombre immobile et muette un frippement étouffé, une plainte éteinte, un soupir ... Puis la salle retombe dans une paix sourde et mystérieuse.Là-bas où une lampe à bec est posée, à côté d'un petit livre de prières, sur une chaise dont elle éclaire la paille, une grosse fille qui a les deux pieds appuyés au bâton de la chaise se lève, les cheveux ébouriffés par le sommeil, du grand fauteuil recouvert avec un drap blanc, où elle se tenait somnolente. Elle passe comme une silhouette, sur la lumière de la lampe, va à un poêle, prend la pointe de fer posée sur la cendre chaude, remue et tracasse deux ou trois fois le charbon de terre, revient à son fauteuil, repose ses pieds sur le bâton de la chaise et s'allonge de côté.Le feu, avivé, rayonne plus rouge. Dans leur godet de verre allongé, pendu à deux branches de fer arrondies, les veilleuses s'éteignent et se raniment. Leur lumignon se lève et abaisse, comme un souffle, sur l'huile lumineuse et transparente. Le fumivore qui se balance à leur flamme mobile, projette sur les poutrelles du plafond une ombre énorme dont le cercle s'agite et remue sans cesse. Au-dessous, à droite et à gauche, la lumière coule mollement, du verre suspendu, sur le pied des lits, sur la bande de toile froncée qui les couronne, sur les rideaux dont elle jette l'ombre en écharpe au travers d'un corps pelotonné sous une couverture. Les formes, les lignes s'ébauchent en tremblant dans le demi-jour incertain qui les baigne, tandis qu'entre les lits, les fenêtres hautes, mal voilées par les rideaux, laissent passer la clarté bleuâtre d'une belle nuit d'hiver sereine et glacée.De veilleuse en veilleuse, la perspective s'éloigne, les images s'effacent et se confondent. Aux endroits où la clarté de l'une cesse et où la clarté de celle qui suit ne luit pas encore, de grandes ombres noires se lèvent toutes droites et se joignent au plafond, mettant la nuit aux deux côtés de la salle. Au-delà, l'oeil perçoit encore une confuse blancheur; puis la nuit revient, une nuit opaque où tout disparaît.
