Durch den medizinischen Fortschritt, verbunden mit einer immer größeren Nachfrage nach Behandlungen, einer negativen demographischen Entwicklung, die immer ältere und damit auch behandlungsbedürftige Menschen mit sich bringt, wird sich die Situation im Gesundheitssystem in den nächsten Jahren noch verschärfen (vgl. Hoppe 2008: 304). Es stellt sich also gerade für die Politik die Frage, wie ein Ausweg aus der Misere zu finden ist. Hauptlösungsvorschläge, die sich in politischen und wissenschaftlichen Diskussion finden, sind (neben der Steigerung von Einnahmen im Gesundheitssystem) die Rationierung, die Rationalisierung und die Priorisierung von Gütern und Leistungen um eine angemessene Reaktion auf die kommende Entwicklung zu finden. Die Frage, mit der sich diese Arbeit beschäftigt ist daher, ob das Konzept der Leistungsbegrenzung notwendig oder sogar unvermeidlich ist, um die Probleme im Gesundheitssystem zu lösen. Um die Kontroversität klar zu machen, sollen zwei Gegenargumente betrachtet werden. Zum einen handelt es sich um die Aussage, dass Rationierung nicht notwendig ist, da auch Rationalisierung die Probleme lösen kann. Die andere Position geht davon aus, dass Rationierung im Gesundheitssystem nicht vorgenommen werden darf, da Gesundheit als höchstes Gut so elementar ist, dass jede erdenklich Form der Leistung erbracht werden muss und eher eine Einsparung in anderen Bereichen in Kauf genommen werden soll.
