About the Book

The aviation industry partitions the root cause of aviation accidents into specific areas when in fact all accidents are attributed to human error. Causes are grouped into the areas of pilot error, weather, maintenance, air traffic control, and the airframe itself. This exercise of grouping is misleading and incorrect. Additionally, the industry collects and segregates other causal factors while segregating additional contributors deemed as contributors, like terrorism. This retrospective analysis examines aviation mishaps for their true root cause. Analysis indicates causal factors should not be segmented into specific area and be focused on human activities. This research exhibits mishaps are caused by human error and recommends new categorization to improve investigation techniques and education to the aviation industry.
About the Author: Prior to entering corporate America, Mr. Stein was a Senior Officer in the Marine Corps specializing in the subject areas of Aviation, Aviation Maintenance and Logistics, and Information Technology. He is a former Director of Safety and Standardization for one of the largest U.S. Marine Aircraft Goups and a former Aviation Chief of Staff for Naval Command Coronado, CA. Since his transition to Corporate America he has held numerous senior managerial and executive positions. Additionally, in a Fortune 50 company, he developed and implemented an entire program development methodology that lead to a consistent, profitable, and thorough approach to program management and development. As an Associate Professor for Embry-Riddle Aeronautical University and a member of the faculty staff since 1998, Mr. Stein instructs and develops programs for undergraduate and graduate students. He is a graduate of the United States Naval Academy and holds Masters Degrees from Naval Postgraduate School, Monterey, California, and Embry Riddle Aeronautical University, Daytona, Florida as well as certificates in Aviation, Education, and Information Technology. As an executive program manager he has held some of the highest achievable positions and designations in his fields of expertise throughout the world. Tom remains active within the military arena as a mentor. He is the former Chairman of the Board for the Orange County Veteran Employment Committee and presently the President and Founder of 'THE HIRE TARGET, ' a non-profit organization focusing on assisting Military Families with their transition from military to civilian careers.
