Resultante à guerra constante nas fronteiras e às defesas de posições contra vários grupos, os soldados romanos entraram em contato com doenças estrangeiras, e eles involuntariamente os trouxeram para casa quando as campanhas terminaram. Isso culminou por volta de 165 d.C., quando uma doença não identificada causou o império de joelhos e afligiu um número incontável de desejados, um dos quais pode ter sido Lúcio Verus, ou co-imperador de Roma ao lado de Marco Aurélio.
Além do enorme número de matança, que foi estimado em mais de 5 milhões de pessoas, uma pandemia interrompeu o comércio romano a leste, afetou sociedades culturalmente em toda a Europa, e obrigou médicos como Galeno a estudar os sintomas em um esforço para descobrir não apenas se houver doença, mas quaisquer curas potenciais. Claro, essa foi uma tarefa alta para médicos com tecnologia relativamente primitiva, e até hoje como as pessoas continuam a debater sobre que uma doença era e de onde ela veio, com teorias que vão desde um surto de varíola na China, ou possivelmente sarampo.
