La médecine occidentale considère que la maladie de Parkinson résulte d'un déficit de la production de dopamine cérébrale dans la zone de la substance noire. Il en découle la thérapeutique qui consiste à compenser cette pénurie par un apport extérieur d'un ou plusieurs médicaments dopaminergiques.
Les observations au microscope de coupes du cerveau de patients décédés ont montré que cette baisse de la production était en rapport avec une diminution du nombre de neurones producteurs.
La recherche des causes de la diminution de la production de dopamine a découvert un certain nombre de facteurs responsables d'une dégradation structurelle. L'agression peut être externe (toxiques, virus, traumatisme...) Ou avoir une origine interne (tumeur, embolie, hémorragie...) ou être d'origine inconnue (idiopathique).

La Dr Janice Hadlock. A découvert une cause relativement fréquente de dégradation fonctionnelle du système dopaminergique qui régule les mouvements. Elle pense que c'est la cause de la maladie de Parkinson idiopathique.
Elle a appelé cet état le mode pause . Cet état physiologique que l'on peut observer dans le règne animal de la fourmi jusqu'aux mammifères que les biologistes décrivent sous le terme de thanatose . C'est un état de paralysie musculaire utilisé en dernier recours pour leurrer un prédateur. Il est automatiquement réversible dès que le danger est passé.
C'est la persistance des effets de cet état qui n'a pas été complètement réinitialisé qui serait la cause du déficit en dopamine dans la zone cérébrale du mouvement.
Cet état est également décrit par les pédiatres sous le nom de réflexe de paralysie par la peur. Nous en reparlerons à propos des réflexes de survie.
Il est aussi connu par la sagesse populaire qui a remarqué qu'une grosse frayeur peut couper les jambes, avoir les jambes en coton, cloué sur place, glacé, pétrifié, scotché, ou encore faire trembler comme une feuille.
Dans les cas de Parkinson secondaire à un réflexe de paralysie par la peur, les cellules productrices de dopamine de la substance noire ne sont pas mortes elles sont mises en veilleuse et elles attendent le signal que la situation soit sûre pour reprendre leur pleine activité.
Seuls les cas de maladie de Parkinson secondaire à une destruction des neurones (toxines, drogues, accidents vasculaires, sont en déficit pathologique de dopamine.
