Zu dem entschiedensten Einschnitt zählt dabei der stetige Anstieg der Gesundheitsausgaben. Somit war es immer oberstes Ziel der Gesundheitspolitik diese Ausgaben zu begrenzen. Um dieses Ziel erfolgreich zu erreichen, war es ein relevanter Schritt die Einnahmen der gesetzlichen Krankenversicherungen mit der gesamtwirtschaftlichen Entwicklung des Gesundheitssystems zu verbinden. Aufgrund dessen, dass die Mehrheit der deutschen Bevölkerung gesetzlich versichert ist, "bilden die stabilen Beitragsätze der gesetzlichen Krankenversicherungen das wesentliche Element der gegenwärtigen Gesundheitspolitik". Gerade unter diesem Gesichtspunkt kam es in der Vergangenheit zu häufigen Reformen und Gesetzgebungen im Gesundheitswesen. Um neue Wirtschaftlichkeitspotentiale erschließen zu können, welche die Ausgaben reduzieren und eine Erhöhung der Beiträge verhindern sollen, ist die Politik durchaus gewillt, den Gesundheitssektor noch wettbewerbsorientierter auszurichten. Im besonderen Fokus der Reformgesetzgebung steht dabei der Krankenhaussektor. Dieser bildet bei den Krankenversicherungsunternehmen den größten Kostenblock. Um dieser Entwicklung entgegenzuwirken, wurde 2003 das deutsche DRG-basierende Vergütungssystem eingeführt. Die Gesundheitsleistungen sollen zukünftig rein auf Basis von leistungsbezogenen und pauschalisierenden Fallgruppen (Diagnosis Related Groups -DRGs) finanziert werden. Für die Krankenhäuser bedeutet dies nichts anderes, als dass ihre erbrachten Leistungen durch neue vorher gesetzlich definierte Budgets und Preise bemessen werden.
