Dorothea Brooke er en 19-årig forældreløs, der bor sammen med sin yngre søster, Celia, som en menighed af hendes onkel, Hr. Brooke. Lydgate har nye ideer om medicin og sanitet og mener, at lægerne skal ordinere, men ikke selv udlevere medicin. Dette trækker ire og kritik af mange i byen. Han allierede sig med Bulstrode, en velhavende, kirkegående godsejer og udvikler, der ønsker at bygge et hospital og en klinik, der følger Lydgates filosofi, på trods af betænkelighederne hos Lydgates ven, Farebrother, om Bulstrodes integritet. Lydgate bliver også bekendt med Rosamond Vincy, hvis skønhed og uddannelse går sammen med overfladiskhed og selvoptagethed. Hun forsøger at finde et godt match og beslutter sig for at gifte sig med Lydgate, som kommer fra en velhavende familie, og bruger Freds sygdom som en mulighed for at komme tæt på lægen. Lydgate i første omgang ser deres forhold som ren flirt og bakker væk fra Rosamond efter at opdage, at byen anser dem praktisk engageret. Men efter at have set hende en sidste gang, bryder han sin beslutning, og de to bliver forlovet.Casaubon ankommer tilbage fra Rom omkring samme tid, kun for at lide et hjerteanfald.
