En raison des frissons sévères de nombreux patients, les médecins espagnols ont d'abord typé le paludisme ou la typhoïde abdominale. Les survivants étaient souvent marqués par une fatigue intense et un épuisement chronique pendant des semaines, et il n'était pas rare qu'une dépression se produise en conséquence. Ceux qui ont survécu à une pneumonie ont souvent été confrontés à une convalescence longue et difficile. À la suite de l'infection grippale, de nombreuses personnes ont souffert de dysfonctionnements neurologiques pour le reste de leur vie, comme l'ont décrit les pathologistes à plusieurs reprises décrit l'encéphalite haemorrhagica acuta, à l'époque appelée encéphalite aux puces, et ses conséquences. La grippe asiatique (1957) et la grippe hongkongaise (1968) étaient basées sur d'autres sous-types, mais la majorité des gènes internes proviennent du virus de la grippe espagnole, c'est pourquoi il a été désigné en 2006 comme la "mère de toutes les pandémies". Une augmentation notable des cas d'encéphalite léthargique a également été observée. Il s'agit d'une forme d'inflammation du cerveau qui provoque une léthargie, des crises de sommeil incontrôlées et un trouble temporaire semblable à la maladie de Parkinson et, dans certains cas, au parkinsonisme post-céphalitique permanent. Cependant, un lien direct entre l'encéphalite léthargique et la grippe espagnole n'a pas été établi; par McCall et al. en 2001 et Lo et al. Les échantillons de tissus examinés en 2003 n'ont trouvé aucune preuve du virus de la grippe.
