Die Wahlmöglichkeiten und der Umgang mit der Realität werden nach dem therapeutischen Selbstverständnis des Brügger Modells durch die Faktoren Erfahrungen, Erziehung, genetische Faktoren und den Kontext bestimmt. Dabei wird zwischen der semantischen und der pragmatischen Wahl unterschieden: Unsere Wahrnehmung der Realität wird geleitet durch die Wahl der Zuschreibung von Bedeutungen (semantischer Aspekt), die unser Handeln (pragmatischer Aspekt) leiten. Ist die Bedeutungswahl in der Wahrnehmung eingeschränkt, so ist auch die pragmatische Wahl eingeschränkt Die Aufgaben des Therapeuten im Brügger Modell beziehen sich darauf, einen Kontext herzustellen, in dem gemeinsam mit dem Patienten Möglichkeiten der semantischen und der pragmatischen Wahl entwickelt werden, um auf diesem Wege die Wahlfreiheit wieder herzustellen oder zu erweitern: Der (Wieder-)Herstellung von Wahlfreiheit dient die Veränderung von (pathologischen) Gewohnheiten. Da die wenigsten Gewohnheiten stets völlig gleich ablaufen - selbst bei Zwangsritualen finden sich oft kleine Varianten -, bieten sich hier therapeutische Chancen. Die Gewohnheiten und ihr Vollzug werden im therapeutischen Gespräch und in Selbstbeobachtungsaufgaben des Patienten erkundet. Daraus wird ein vom Patienten gewünschter Zustand abgeleitet sowie Wege, diesen Zustand zu erreichen. Die Annahme, dass Patienten bei wieder hergestellter Wahlfreiheit die Wahl treffen, anstelle ihres pathologischen Verhaltens ihre Symptome zu beherrschen, ist aus der therapeutischen Erfahrung abgeleitet.
