Le traite, dont le titre originel est inconnu, est l'un des fleurons les plus remarquables du rationalisme hippocratique. Malheureusement il a ete victime, des la haute Antiquite, d'une dislocation accidentelle au cours de la transmission du texte. Traite unique ecrit par un seul et meme medecin, il a ete transmis en deux livres separes intitules Epidemies I et Epidemies III, alors que cette dislocation ne correspond a aucune division majeure du traite originel, les deux parties devant etre remises bout a bout pour reunir ce qui etait deja separe au temps de Galien. C'est l'oeuvre d'un medecin qui a ete un maitre de l'observation de la realite quotidienne du malade qu'il a confiee a l'ecriture. C'est, en effet, le traite le plus ancien ou apparaissent des fiches de malades decrits au jour le jour de la maladie. On dispose ainsi d'une totalite de quarante-deux fiches de malades auxquelles il faut ajouter les noms de vingt-six malades particuliers cites en exemple au cours d'un tableau nosologique. Ce medecin a exerce particulierement dans la Grece du Nord a Thasos, mais aussi dans d'autres cites de la cote thrace ou de Thessalie. C'est ce qui ressort de l'examen des fiches ou le medecin presente dans nombre d'entre elles non seulement des precisions sur l'identite du malade, mais aussi des indications topographiques, sur la cite ou il a examine le malade, voire sur son adresse par reference a un lieu connu de la cite.

A cette observation du detail guidee par tout un savoir medical implicite sur la classification des maladies et sur la signification diagnostique et pronostique des signes pathologiques, l'auteur ajoute un sens remarquable de la synthese, guide par la conviction que les maladies suffisamment repandues dans la population d'une cite pour avoir le statut de maladies epidemiques s'expliquent dans le cadre environnemental des quatre saisons de l'annee. C'est a l'interieur de ce cadre, la constitution climatique de l'annee, que le medecin presente quatre tableaux nosologiques dans une cite donnee, en l'occurrence Thasos (au moins pour les trois premieres). Ces comptes rendus climato-nosologiques dresses dans une cite particuliere sont, comme les fiches de malades, une grande premiere dans l'histoire de la medecine occidentale. Ils inaugurent l'emergence d'une medecine statistique avant la lettre sur la proportion des malades atteints par les differentes maladies au cours de chaque saison.En troisieme lieu, a la description du detail et a la vision d'ensemble s'ajoute un volet reflexif sur l'art de la medecine, ou le medecin prend de la hauteur apres les tableaux nosologiques et avant les fiches de malades. La fulgurance de certaines formules etonne. C'est en particulier le cas de trois considerations a la fin de la deuxieme constitution (c. 11 2) qui sont restees parmi les plus celebres de la collection medicale attribuee a Hippocrate: 1. sur la necessite du pronostic dans les trois dimensions du temps: Dire le passe, comprendre le present, predire l'avenir .2. sur la finalite de l'action du medecin: pratiquer, a propos des maladies, deux choses: etre utile ou ne pas nuire .3. sur la definition de l'art de la medecine: L'art a trois termes: la maladie, le malade et le medecin.
