Secondo i dati ISTAT 2015, in Italia soffre di diabete mellito il 5,4% degli italiani (sia tra i maschi che tra le femmine), per un totale di oltre 3 milioni di persone.
Le zone in cui la prevalenza del diabete è maggiore sono le regioni del Meridione, in particolare la Calabria.

Il diabete, o meglio diabete mellito, dipende dall'insulina.

Per la precisione ci può essere:
-una ridotta disponibilità di insulina, quindi produzione insufficiente all'organismo per il suo corretto funzionamento;
-una scarsa sensibilità all'ormone da parte dei tessuti bersaglio, ovvero l'insulina è presente, ma l'organismo non riesce a farne buon uso;
-la combinazione di questi fattori, cioè l'insulina è poca e non funziona in modo adeguato.

La conseguenza misurabile è l'iperglicemia dovuta alle suddette alterazioni a carico dell'insulina. Oggi vengono distinti: DIABETE DI TIPO 1 DIABETE DI TIPO II DIABETE GESTAZIONALE
Il diabete nella popolazione mondiale è aumenta negli ultimi 30-40 anni tanto che si parlava di 108 milioni nel 1980 contro i 422 milioni del 2014. Si tratta di una malattia metabolica dovuta a un calo di attività dell'insulina, ormone prodotto dalle cellule beta delle isole di Langerhans del pancreas.
La conseguenza è l'iperglicemia cioè l'eccessiva concentrazione di glucosio nel sangue. Con il passare del tempo, tende ad essere seguita da complicanze di natura vascolare, come:
macroangiopatia (forma di aterosclerosi particolarmente grave e precoce).

microangiopatia (alterazione della circolazione sanguigna dentro i piccoli vasi arteriosi, in particolare nella retina, nel rene e nei nervi). Alterazione specifica del diabete
CLASSIFICAZIONE
La classificazione internazionale del diabete mellito risale al 1997 identifica tre tipologie principali:
DIABETE MELLITO DI TIPO 1. Raccoglie la quasi totalità di forme diabetiche immunomediate. La causa di fondo è quindi un malfunzionamento del sistema immunitario che riconosce come estranee le cellule beta pancreatiche delle isole di Langerhans, produttrici di insulina, le aggredisce e le distrugge. Si tratta quindi di una malattia autoimmune.
DIABETE MELLITO DI TIPO II. Raccoglie tutte le forme dovute a un deficit di secrezione dell'insulina, da parte delle cellule beta pancreatiche delle isole di Langerhans. Si ha anche resistenza dei tessuti dell'organismo all'azione dell'insulina fenomeno detto insulino-resistenza.
DIABETE GESTAZIONALE. Comprende le forme di diabete secondarie allo stato di gravidanza. In genere, è un fenomeno transitorio.
