En el Capítulo 2 Definiciones, se incluyen definiciones de distintos términos que se requerirá conocer a lo largo del libro. En el Capítulo 3 Introducción, se presenta un ejemplo de análisis de incidentes en la minería, identificando la causa raíz, las lecciones aprendidas y las acciones correctivas. Presentamos los principales conceptos en que se basan los sistemas corporativos de gestión de riesgos: -la rueda de Nertney, -el ciclo de vida de los componentes de un proceso, y -la tolerabilidad del riesgo. Además nos referimos a las normas de Normas de Gestión de Riesgos NZ/AS 4360, e ISO 31.000:2009. El Capítulo 4, Consecuencias al no usar Gestión de Riesgos, presentamos: -Ejemplos en minería que han originado grandes pérdidas de vidas humanas y al negocio, debido a falta de una gestión efectiva de riesgos. El Capítulo 5, Procesos de la gestión del Riesgo, es el más extenso y presentamos: Los distintos Procesos de la Gestión de Riesgos establecidos en la Norma ISO 31.000:2009, desde "Establecer el Contexto" hasta el "Tratamiento de los Riesgos". -El concepto de la "Energía", que es un método aceptado en minería para el proceso de identificación de los peligros. -Diversos métodos para el análisis y evaluación de riesgos, especialmente los métodos de análisis cualitativos, debido a que es la forma más simple para el análisis. Mostramos también algunas técnicas semi cuantitativas y cuantitativas, aunque la industria minera no ha avanzado tanto en la aplicación de ellas como la industria del petróleo, que es también otra industria de alto riesgo. -Resaltamos la importancia del Análisis Preliminar de Riesgos (PHA). Una vez identificado en PHA un riesgo principal (Top Risk), se recomienda aplicar posteriormente la técnica Bow Tie (BTA), con el objetivo de definir los controles o barreras que sean necesarias. -El Error Humano es también importante, pues el análisis de riesgos cuantitativo debiera considerar el error humano como otra fuente de información para el diseño de equipos, de manera de evitar incidentes en la interface de las personas con los equipos. -El concepto de la "Jerarquía de Control", que es normalmente utilizado en minería para evaluar la efectividad de los controles. -La Tolerabilidad del Riesgo. Si un peligro está potencialmente presente, existe riesgo, y en consecuencia debe ser gestionado a un nivel tan bajo como razonablemente práctico (ALARP). -Los Cambios. Generalmente las organizaciones en minería tienen un procedimiento formal para administrar los cambios, reconocen que el cambio puede generar potencialmente un riesgo, y a eso se refieren también los procesos de "monitoreo y control" de la Norma ISO 31000:2009. En el Capítulo 6, se describe el Método de HAZOP (Hazard Operability), técnica que ha sido ampliamente adoptada en la minería, los organismos legislativos y las autoridades otorgadoras de permisos, para minimizar el riesgo al público, la propiedad y al medio ambiente, durante la puesta en marcha de los proyectos sistemas y operaciones. En el Capítulo 7, proporciono un ejemplo de desarrollo de un Taller Hazop. En el Capítulo 8, se presenta una guía para realizar un análisis Bow- Tie (BTA), e incluyo un ejemplo aplicado. Y en el Capítulo 9, se listan las referencias.
