En la actualidad, hablar de Alzheimer es hablar de una de las enfermedades más crueles e inclementes que el hombre pueda estar observando en los últimos tiempos. Es una enfermedad que, cuando llega, no tiene en cuenta el estatus social y económico de las personas; no tiene en cuenta el modo ni el estilo de vida que hayamos escogido -salvo en muy raras excepciones y de las que vamos a tratar- tampoco respeta los títulos y honores de algunos llamados privilegiados. En ésta efímera existencia, llamada vida: ¿quién no ha conocido o conoce un amigo, un familiar, un cercano, a quien ésta enfermedad no le haya robado lo más precioso que se pueda tener: la identidad y la vida misma? Efectivamente, el Alzheimer es un monstruo que pega y llega fuerte. ¿Cómo comprender lo incomprensible? La enfermedad de Alzheimer (EA) es un verdadero reto a la razón humana. En este sentido, se nos viene a la memoria que cuánto más nuestros conocimientos se acumulan, más el mundo llega a ser incomprensible y lo que más nos hace falta, no es el conocimiento de las cosas que nosotros ignoramos, pero sí nos hace falta la capacidad de pensar en las cosas que nosotros ya conocemos." . Es decir, no es más importante acumular conocimientos para luego no poder aplicarlos, sino por el contrario, hay que hurgar y buscar en lo que ya tenemos, en aquellas "reservas" aprendidas, adquiridas, y reforzar mucho más lo que ya sabemos. De allí que, para el investigador, se asevere de extrema importancia el estudio y desarrollo de la reserva cognitiva (RC) y su interacción con el modo de vida, para hacer frente a ésta epidemia del siglo, como ya algunos la denominan. Por ello, no debemos permitir que el Alzheimer se convierta en una enfermedad implacable. Pues si bien es cierto que, por el momento no podemos destruirla de raíz lo menos que podemos hacer es tratar de aplacarla con nuestras "reservas". Y en este sentido, conviene especificar el concepto de esta palabra: reserva.Etimología de las palabras "reserva" y "cognición" . Connotación científica.La palabra reserva proviene del verbo latín "reservare" que significa "poner de lado", "reservar": "Aliquid in aliud tempus" (Cic. Prov. 47), "reservar algo para otro momento." Igualmente significa "conservar", "salvar." Así, al hablar de reserva nos estamos refiriendo a los recursos que tenemos para resolver una necesidad o para llevar a cabo una empresa. Es la guarda o custodia que se hace de una cosa. En neuropsicología "reserva" es la habilidad que tiene el cerebro para tolerar mejor los efectos de una patología asociada a la demencia; sería el resultado de una habilidad innata o de los efectos de las experiencias vividas, tales como la educación o la ocupación laboral (Manly y cols., 2003). Por otra parte, la palabra cognición proviene de la palabra latina "cognitio"; que a su vez proviene del verbo latín "cognoscere"; que significa la acción de aprender a conocer. En otras palabras: Es la acción de aprender a conocer, a través de la inteligencia . En psicología se entiende el término "cognición" como el conjunto de actividades intelectuales y procesos que se relacionan con el conocimiento y la función que lo realiza. Pero, hablar de cognición y conocimiento no es equivalente. Efectivamente, "cognición" es más general que el segundo. El conocimiento es el objetivo, o la función -en el sentido que la biología confiere a esta palabra- de la cognición. La definición de esta última comprende entonces al mismo tiempo: el conjunto de actividades que conllevan al conocimiento, funcionando de forma correcta o más o menos incorrecta; y el conjunto de cosas que se producen a partir de aquellas actividades, se trate ya sea de conocimientos propiamente dichos o de representaciones y de creencias aproximativas o parcialmente inexactas.
