Questo testo si propone di offrire al lettore una chiave di accesso psicanalitica alla questione dell'uso di sostanze stupefacenti e degli effetti di discorso, di cui deve tener conto una clinica del legame sociale, a partire dall'esperienza clinica maturata nel quotidiano lavoro nei Servizi Sanitari pubblici di "prima linea," oggi in primis i Ser.T, e in particolare nella prima struttura pubblica per la Doppia Diagnosi, e ancora unica in tutta la regione Campania, denominata U.O. Speciale di Comorbilit psichiatrica - Centro Diurno "Giano," diretta dal dott. Gilberto Di Petta, e afferente al Dipartimento Dipendenze Patologiche, diretto dal dott. Vincenzo D'Auria, dell'ASL Napoli 3. Il consumo di droghe rappresenta un dato che deve trovare un posto sul versante dell'ipotesi diagnostica che guida lo psicoanalista nella cura, a partire dalla conclusione di un congruo tempo logico, definibile come preliminare, ma non rappresenta un elemento patognomonico valorizzato al fine di formulare una diagnosi di tossicodipendenza. La diagnosi di tossicodipendente non esiste nel campo freudiano, come ben sottolinea Markos Zafiropoulos, ma non di meno il consumo di sostanze stupefacenti pu trovare un proprio posto nell'economia soggettiva, cio nella strategia del soggetto freudiano, le cui modalit strutturali, quelle che il soggetto pu assumere a seconda di come si pone nei confronti della castrazione, sono la nevrosi, la perversione e la psicosi. Non c' tossicomane, o se vogliamo diagnosi di tossicomania, in questo elenco; non di meno, la clinica psicoanalitica non affatto tagliata fuori dalla possibilit di giocare un ruolo notevole per i soggetti che fanno uso di sostanze stupefacenti. Lacan ha impresso alla clinica un particolare orientamento, che non ha mancato di evidenziare sin dalle prime battute del suo insegnamento: la psicoanalisi, prima ancora di essere una teoria del soggetto e un metodo di cura, rappresenta un certo discorso che non vuole tirarsi indietro nel trattare gli altri discorsi, definiti come forme di legame sociale. E infatti che il tossicomane esista, o ancor di pi che si presenti come un'evidenza, indubitabile per alcuni degli altri discorsi possibili, come per esempio quello medico, quello sociale, quello economico, quello giuridico e, frequentemente, per il discorso dello stesso individuo che fa uso di sostanze. Il testo affronta la questione del trattamento dei soggetti che fanno uso di sostanze e analizza, attraverso gli scritti di Freud, che non c' nessuna logica - per questa ragione che Lacan, pi che di tecnica della psicoanalisi, preferiva parlare di etica - che possa guidare necessariamente un soggetto nell'una o nell'altra direzione: verso il piacere o la realt, verso il dovere o verso il godimento, ma c' del desiderio, ed al trattamento (etica) di questo (desiderio) che la psicoanalisi non si sottrae.
