In Italia il morbo si manifestò in forma epidemica nel 1494, in occasione dell'assedio di Napoli ad opera delle truppe francesi al comando di Carlo VIII, morto all'età di 28 anni per sifilide cerebrale, come riportato dal G. Del Guerra. Era al seguito dell'esercito francese un gruppo di circa 800 prostitute, e non c'è dubbio che proprio la diffusione del meretricio negli eserciti e tra la popolazione contribuisse in massima parte a propagare quella che, nella nostra penisola, fu chiamata "Peste Gallica" o "mal francese", da cui il il termine "infrancesato", o "infranciosato". Rileva il Pazzini (Storia delle malattie): Gli eserciti invasori "avevano a fianco un altro esercito di femmine che diffondevano e propagavano il contagio, sia tra le fila dei soldati che tra la popolazione civile". Nella ricerca spasmodica di un colpevole, la fantasia popolare indicava l'origine del morbo come frutto dell'amplesso di una prostituta con un lebbroso. (Paracelsus medico-alchimista, astrologo, 1493-1541). Colpevoli inoltre di diffondere la malattia, venivano indicati i marrani (ossia gli ebrei convertiti) oppure gli Indios o i marinai provenienti dal Nuovo Mondo. A Roma, verso la fine del '400, si contavano circa 6800 meretrici, escludendo le clandestine. Campo de' Fiori risultava essere il punto centrale ove veniva praticata la prostituzione, ed in una supplica rivolta al Santo Padre, per porvi rimedio, si legge: "In questa città le meretrici incedono per le vie come matrone". A Venezia le prostitute erano obbligate a circolare in pubblico con un fazzoletto giallo intorno al collo, come segno di riconoscimento. Fu l'astinenza sessuale il provvedimento che la Chiesa adottò come rimedio per contrastare tale malattia, ed il Pontefice Paolo IV, intorno alla metà del '500, decretò con un editto la cacciata da Roma e da tutto lo Stato Pontificio delle prostitute. Il malcontento popolare costrinse la Chiesa a trovare una sede dove fosse stato possibile praticare il meretricio, lontano dalla città si decise per una località "Trans Tiberis", oggi Trastevere. All'ingresso di tali postriboli regnava la scritta: "Non proestat foemina morbo infesta", oppure "Hic habitat felicitas". Nel suo "De preservatione a carie gallica" del 1555, Gabriele Falloppia ideò una protezione individuale al mal francese, consistente in una pezzuola di lino a forma di sacchetto "ad mensuram glandis" imbevuta di medicamento a base di mercurio: era l'antesignano del moderno profilattico. A Roma, all' Ospedale San Giacomo degli Incurabili, il Guaiaco, in considerazione dell'alto costo, veniva distribuito gratuitamente ai malati di Sifilide. Affluivano al S. Giacomo centinaia di malati di sifilide da ogni parte del paese ed in un registro dell'accettazione, conservato nell'archivio storico del S. Spirito, nell'anno 1525 sono segnati circa duemila degenti per la cura di questa malattia.
